Hormontherapie

Eine transfeminine Hormontherapie dient in der Regel Feminisierung und Demaskulinierung des Körpers, sowie Reduzierung von Gender Dysphoria. Dazu können Östrogene, Progesterone und Anti-Androgene eingesetzt werden.

Eine häufig benutzte Richtlinie für die Hormonwerte sind 100-200pg/ml Östradiol und Testosteron unter 50ng/ml. Bei einer Monotherapie sind oft höhere Östradiolwerte ein Ziel, denn über 200pg/ml kann Testosteron in der Regel auch ohne Antiandrogene unterdrückt werden. (Aly, 2018/2025)

Die Effektivität von HRT-Medikation ist sehr individuell und es ist wichtig, dass regelmäßig Blutwerte gemessen werden, um individuelle Entscheidungen über Wahl der Medikation und Dosis zu treffen.

Estradiolvalerat-Injektion

Injektionen (Valerat)

  • Dosis:
    • 4mg/5 Tage (Transfeminine Guide: Dosing, 2024)
    • 5mg/7 Tage (bzw. 1-6mg/7 Tage, eher höher wenn ohne Antiandrogen), bzw. Dosis anpassen für alle 3-7 Tage (Aly, 2021/2025)
      • 1-6mg/7 Tage ist für transfeminine HRT besser geeignet als viele Guildelines (mit bis zu 80mg für zweiwöchentliche Injektionen). Höhere Östradiollevel können dann nützlich sein, wenn Testosteron ohne Anti-Androgen unterdrückt werden sollte (dafür reichen 200-300pg/ml). Ein wöchentliches Intervall (oder kürzer) ist für stabile Level zu bevorzugen. ("[1-6mg/weekly] is likely to be more appropriate for use in transfeminine people than current recommendations by many guidelines. Although high estradiol levels can be useful in transfeminine hormone therapy when antiandrogens are not used due to their greater efficacy than physiological levels in terms of testosterone suppression, only modestly supraphysiological estradiol levels (e.g., ~200–300 pg/mL or 730–1,100 pmol/L) appear to be required for strong testosterone suppression [...] [estradiol valerate appears] to produce similar average estradiol levels (e.g., around 300 pg/mL or 1,100 pmol/L at a dosage of 5 mg/week).       • Die Guidelines sind oftmals nicht transparent wie eine Dosis-Empfehlung zustande kommt und die Werte decken sich häufig mit veralteten Studien ohne Blutwertmessungen oder mit Studien von geringer Qualität.

      • Vor dem Hintergrund des sehr weiten Spektrum an typischen Dosierungen 2-20mg/7 Tage, kam dieses retrospektive Chart-Review von 459 erwachsenen Transfems, die 3-4mg/7 Tage Estradiolvalerat injizieren, zu dem Ergebnis, dass diese vergleichsweise niedrige Dosis meistens ausreiche. 45.8% nahmen zusätzlich Spironolacton, 18.3% Progesteron, 5.9% Finasterid/Dutasterid. 79.5% derjenigen ohne Spironolacton erreichten Testosteronlevel unter 50 ng/dL, was die Studie als "Monotherapy" bezeichnete. Leider werden nur die jeweiligen Anti-Androgene direkt verglichen, nicht mit einer Referenzgruppe ohne Anti-Androgene.
      • Es lässt sich aus den Daten ableiten, dass eine Monotherapy Gruppe (mindestens 30% des Protocol Subset) durchschnittlich Testosteronwerte von ca. 53ng/dL hatte, was auch mit dem Subset von Regeimens mit Progesteron übereinstimmt. Es scheint aber einige Outlier zu geben und der Medianwert lässt sich nicht ableiten. Insgesamt sind die Daten teilweise nicht statistisch signifikant.

      • Diese Studie untersuchte die Wirksamkeit von regelmäßigen wöchentlichen Estradiol-Valerat (93%) und Estradiol-Cypionat (7%) Injektionen unter 357 erwachsenen Transfems, die noch keine Gonadektomie hatten oder GnRH-Agonisten nahmen. 68% nahmen Spironolacton, 11% Finasterid, 1.7% Bicalutamid. 33% nahmen Progesteron, 9% ein anderes Progestin. Der Median der Estradiol-Konzentrationen war 232 pg/mL (50% zwischen 134-371). Unter denjenigen mit erfolgreicher Testosteronunterdrückung (82.3%, <50ng/mL) war die Estradiol-Konzentration 246 pg/mL (50% zwischen 151-379), ohne 169 pg/mL (50% zwischen 79-277). "Patients using estradiol monotherapy were compared to those using estradiol with an antiandrogen(s) or progestogen [...] those who reached TT suppression versus those who did not, the 2 groups were largely comparable, except for a higher median estradiol concentration and dose in those who reached TT suppression (246 vs 169 pg/mL and 5 vs 3 mg, respectively). There was a negative correlation between TT concentration and estradiol concentration [...] progestogen use was independently associated with a lower TT concentration. Spironolactone, type of ester (EC vs EV), and route of estradiol administration (IM vs SC) did not significantly affect TT concentration."

  • Orale Aufnahme hat ein stark erhöhtes Risiko für Blutgerinsel. Wir empfehlen es daher nicht.

    • "A retrospective cohort study in the United States found that the incidence of thromboembolism in transfeminine people with an average dose of 4 mg/day oral estradiol was approximately twice that of cisgender controls not taking hormone therapy [...] such increases can significantly increase morbidity and mortality. A 2021 meta-analysis reported an absolute incidence of VTE of 2% in transfeminine people receiving gender-affirming hormone therapy, although with significant between-study heterogeneity." Estradiol-Spray

  • Estradiol Spray ist eher ineffektiv. Wir empfehlen es daher nicht.
    • "Estradiol sprays, such as Lenzetto, have been found to achieve very low estradiol levels that are probably not therapeutically adequate for use in transfeminine hormone therapy" (Aly, 2018/2025, graph)

Anti-Androgene:

Cyproteronacetat

  • Anwendung: Cyproteronacetat ("CPA") wird in der Regel mit einer täglich Dosis von 5-10mg oral angewendet (geschluckt). Die 10mg Androcur Tabletten wurden eingestellt. Um 50mg Androcur-Tabletten sinnvoll zu dosieren sollten Tabletten halbiert oder geviertelt werden und die Dosis auf 2-3 Tage aufzuteilt werden (z. B. ein Viertel 50mg = 12.5mg, alle zwei Tage entspricht 6.25mg täglich). CPA hat eine Halbwertszeit von ca. 1.5-4 Tagen. Die Dosis kann anhand von Bluttests angepasst werden.

        • "Although doses of CPA of 50 to 100 mg/day have been used in transfeminine people historically, it is now clear that 5 to 10 mg/day CPA has maximal or near-maximal effectiveness in terms of suppression of testosterone levels. [...] [50mg tablets] can be split with a pill cutter and taken once every day to once every other day to achieve an overall CPA dosage of 6.25 to 12.5 mg/day. [...] This lack of additional testosterone suppression with a doubling of dosage [from 5mg to 10mg and 10mg to 20mg] within studies suggests that testosterone suppression with CPA might have actually been maximal at a dosage of only 5 or 10 mg/day. [...] Accordingly, Meyer et al. (2020) assessed a dosage of CPA in combination with estradiol in 155 transfeminine people and found no difference in testosterone levels with 10, 25, or 50 mg/day CPA; testosterone levels were strongly suppressed with all three doses. [...] Even doses of CPA lower than 5 mg/day (e.g., 2 mg/day) may be usefully effective for testosterone suppression if combined with sufficient levels of estradiol [...] Start with the minimum dosage [1/2 a 10mg tablet or 1/8 of a 50mg tablet] of CPA for one month. After one month, have testosterone levels tested and confirm that they are in the normal female/castrate range (<50 ng/dL). Regardless of dosage, a concomitant minimum estradiol level of around 65 pg/mL needs to be attained in order to allow for complete suppression of testosterone levels with CPA. [...] Recommended doses of CPA for physiological progestogenic effects in transfeminine people: 2.5mg/day [...]" (Aly, 2019/2025)

  • Risiko & Nebenwirkungen:
    • CPA scheint ein dosisabhängiges, kumulatives Risiko für Meningeomen (gutartige Gehirntumore) zu haben. Es ist daher nur in niedrigen Dosen (<10mg) empfohlen und eignet sich gut als Übergang bis zu einer OP.
      • "There was a total of 165,988 subjects with usage of CPA. [...] Usage of CPA is associated with increased risk of meningioma at high doses but not when low doses are also included. Routine screening and meningioma surveillance by brain MRI offered to patients prescribed with CPA is likely a reasonable clinical consideration if given at high doses for long periods of time."       • "Meningiomas were more frequent in older patients and after exposure to CPA (13/103, 13%) compared to NA (1/22, 4%) or CMA (1/85, 1%; P = 0.005). GnRH-Analoga

  • Anwendung: GnRH-Analoga ("GnRHa") werden in der Regel monatlich bis halbjährlich unter die Haut (subkutan) gespritzt, z. B. in Bauch, Gesäß oder Oberschenkel. Typische Ausführungen sind eine Fertigspritze mit Implantat oder eine Zweikammerspritze, mit der kurz vor der Injektion eine Suspension (Injektionslösung) hergestellt wird. Die Injektion erfolgt in der Regel durch eine Ärztin oder einen Arzt.
    • "injected subcutaneously or intramuscularly once per day or once every one to six months depending on the formulation employed.   • GnRHa werden hauptsächlich zu subkutanen Injektionen verwendet.
    • Einige GnRH-Analoga gibt es auch als niedriger-konzentrierte Lösungen zur täglichen Injektion. Diese werden hier ausgelassen, da sie deutlich weniger praktikabel sind als die monatlichen bis halbjährlichen Injektionen. Einige wenige GnRH-Analoga werden auch als Nasenspray (Nafarelin unter den Namen SYNARELA 195,36€/40 Tage bei 0.4mg/Tag, SYNAREL 198,79€/40 Tage bei 0.4mg/Tag und Buserelin unter dem Namen Metrelef) oder Tabletten (Relugolix unter den Namen Orgovyx 6,20€/30 Tage) vermarktet. Allerdings hat sich diese Nutzung bisher deutlich weniger durchgesetzt und wird aufgrund mangelnder Informationslage nicht auf dieser Seite empfohlen.
    • In Deutschland werden fast ausschließlich GnRH-Agonisten verwendet: Vorrangig Leuprorelin (3.6-45mg, 1-6 monatlich) als Implantat oder Suspension verfügbar, gefolgt von Triptorelin (3,75-11mg, 1-6 monatlich), und eher selten auch Goserelin (6,3-9,75mg, 2-3 monatlich) oder Buserelin (3,6-10,8mg, 1-3 monatlich) als Implantate. Der einzige vermarktete GnRH-Antagonist als Suspension ist Firmagon (80-120mg, monatlich).

  • Besonderheiten: Implantate haben dickere Nadeln (14G/2.11mm bis 16G/1.65mm) als Suspensionen (19G/1.07mm bis 23G 0.64mm)

  • Risiko & Nebenwirkungen: GnRHa sind sehr effektiv und risikoarm. Bei GnRH-Agonisten kommt es in der ersten Woche zu einem kurzfristig erhöhten Testosteronspiegel. Häufige Nebenwirkungen sind Reaktionen an der Injektionsstelle (Schmerzen, Verhärtungen, Rötungen, Schwellungen, Juckreiz) und typisch zu erwartende Symptome von erhöhtem bzw. niedrigen Testosteronspiegel.

    • In der ersten Woche steigen Testosteronwerte kurz an.
      • "GnRH agonists cause a brief flare in testosterone levels at the start of therapy prior to the GnRH receptors in the pituitary gland becoming desensitized. Testosterone levels increase by up to about 1.5- to 2-fold for about 1 week and then decrease thereafter"
    • "Hautreaktionen an der Injektionsstelle (z. B. Verhärtungen, Rötung, Schmerzen, Abszesse, Schwellungen, Ödeme, Juckreiz, Knoten- und Nekrosenbildung), die sich in der Regel auch bei fortgesetzter Behandlung zurückbildeten"
      • Ansonsten durch kurzen Anstieg von Testosteron typische Symtome (Knochenschmerzen, Erschwertes Wasserlassen, Muskelschwäche, Schwellungen, Taubheitsgefühle), durch gesunkenen Testosteronspiegel typische Symtome (Hitzewallungen, Knochenschmerzen, Muskelschwäche, Schwitzen, Gewichtszunahme, Müdigkeit) (Verpackungsbeilage Leuproreline)
    • Alle GnRH-Analoga sind ähnlich effektiv und risikoarm:
      • "There is no conclusive evidence to suggest that any one GnRHa is more effective or has fewer side effects than another for the treatment of prostate cancer" (NHS, 2025)
      • "[leuprolide acetate and triptorelin acetate] preparations have not been directly compared in randomized trials, but appear to be similarly effective in suppressing the HPG axis" (Aka, 2005)
      • "A NICE new medicine evidence summary for triptorelin SR states that the evidence on differences in adverse effects [...] among the agents within each class is limited and does not suggest that one agent is superior to the others." (NHS, 2017)

Bicalutamid

  • Anwendung: In der Regel werden zwischen 25-50mg täglich oral angewendet (geschluckt), manchmal auch 25-50mg 2x die Woche. Bica hat eine Halbwertszeit von ca. 6-10 Tagen.
  • Risiko & Nebenwirkungen: Bica kann in seltenen Fällen zu einer fatalen Lebervergiftung führen, daher sind häufige Leberfunktionstests vor allem in den ersten 6 Monaten absolut notwendig. Mögliche Nebenwirkungen, die auf eine Lebervergiftung hinweisen sind: Übelkeit, Erbrechen, Bauchschmerzen, Müdigkeit, Appetitlosigkeit, grippeähnliche Symptome, dunkler Urin und Gelbsucht (Gelbfärbung der Haut/Augen).
    • "To date, there are 10 published case reports of serious liver toxicity in association with bicalutamide. All of these cases were in men with prostate cancer and all occurred within 6 months of initiation of bicalutamide therapy, with two of the cases resulting in death [...] the true numbers may be much greater [...] These instances are merely co-occurrences, and causality in terms of bicalutamide and liver toxicity has not been established. [...] There is additionally an unpublished case anecdote of death in a young transfeminine person associated with bicalutamide [...] she is said to have been a 20-year-old transgender girl in Texas taking bicalutamide with rapid-onset liver failure and no warning signs. Spironolacton

  • Anwendung: Spironolacton ("Spiro") wird in der Regel mit einer täglichen Dosis von 200-400mg oral angewendet (geschluckt). Es empfiehlt sich die Dosis auf 2x täglich aufzuteilen. Spiro hat eine effektive Halbwertszeit von weniger als einem Tag (1.4-15 Stunden).

  • Risiko & Nebenwirkungen: Spiro gilt als risikoarm, aber nebenwirkungsreich. Es kann zu einem erhöhten Kaliumspiegel, verminderten Natriumspiegel und verminderten Blutdruck führen. Häufige Nebenwirkungen sind: Häufiges Wasserlassen, Dehydrierung, Müdigkeit, Schwindel, Übelkeit, Unwohlsein, Parästhesie (Kribbeln). Seltenere Nebenwirkugnen sind: Trockene Haut, Ausschlag und Muskelkrämpfe.
